Participant demographics

We collect data on individuals who participate in our workshops. The first page of this document is the demographics form for all participants EXCEPT teachers. The second and third pages comprise the demographics form for teachers. Demographics forms will be completed during registration on Tuesday.

Daily evaluation forms

This document contains the evaluation forms for Wednesday and Thursday. Each will be filled out at the end of the day.

Final evaluation

This evaluation form will be completed at the end of the workshop on Friday, August 4. The attached document consists of two separate evaluations: the first half (first 3 pages) is the evaluation form for teachers, while the second half (last 3 pages) is the evaluation form for all other workshop participants.
